The following AKC forms have been recreated in Microsoft Word to be a duplicate of the AKC pdf files. The DJAA takes no responsibility for the authenticity of these forms. We will attempt keep these up to date as AKC makes changes to them.

Click on the link below while holding down the Shift key to open the Word file in a new window. When given the option, save it to your pc or open it and then save it.
